• Please use professional names only (omit titles such as Dr., Lt., etc.).
• Include the organization or affiliation, if known.
• Only Suggestion 1 is required. Suggestions 2 and 3 are optional.
If you only have one recommendation, complete Suggestion 1 and submit.
Each member may submit one response.

Submissions will be reviewed by the Conference Planning Committee and Board. Final speaker selection remains at the discretion of FPA leadership.
